By signing this form I waive all claims against Endeavor E-TECHS, its employees or officers in the event of data loss or damages to my computer system. I further certify that I am the owner of this computer and I authorize the Endeavor E-TECHS staff to provide the service requested on this form for the billing rates as provided by Endeavor E-TECHS. All prices and rates are subject to change without notice. Endeavor E-TECHS does not assume any liability or warranty in the event that the manufacturer warranties are voided. I understand that technical support rendered by Endeavor E-TECHS may void manufacturer warranties for this computer system.

Customer will be notified when the specified work being performed has been completed and equipment is ready to be picked up at Endeavor’s business office. In the event equipment is not picked within 180 days after notification the repairs have been completed the equipment will be sent to a recycling company to be recycled.


 

Limitation of Liability  – In no event shall Endeavor E-TECHS or its employees be liable for lost profits or any special, incidental, punitive, indirect or consequential damages arising out of or in connection with the service and/or repair described above, including but not limited to lost revenue, lost profits, replacement goods, loss of technology, rights or services, loss of data, or interruption or loss of use, even if you advise us of the possibility of such damages, whether arising under a theory of contract, tort (including negligence), strict liability or otherwise. Endeavor E-TECHS’ total limit of liability shall be the total cost of the services provided.
