I have read and understand the Donation Policy and Guidelines provided by Endeavor Communications. If selected as a recipient of any Donation Funds, I give full permission to use my name, the name of the organization seeking assistance, along with any given project name in any form of public press or media attention. I understand completing this form is not an agreement, but a request for monetary assistance. By completing this form, I understand that Endeavor Communications reserves the rights to refuse any donation request at their discretion. In the event that Endeavor donates an item, it is to be used strictly for charitable purposes through auctions, door prizes, and raffles, etc., and is not for resale.
